As the first day of school approaches, please request any school forms, medication permission forms, seizure action plans, or other required paperwork for accommodations as soon as possible. To help expedite processing, email forms w/ all your child's personal data filled out directly to your provider's Medical Assistant. You can find their email addresses under the patient resources tab→our staff, then click the name of the Medical Assistant you are trying to reach. Turn around time for forms is 5-7 business days, forms are completed as received, no exceptions. Thank you!
