Thank you for scheduling with Texas Child Neurology. Your forms help us get to know your child before you ever walk in, so your first visit is about your child, not paperwork.
How it works

Watch your inbox
Our front office emails your complete new patient forms one week before your appointment, so you have plenty of time to fill them out in detail without feeling rushed.

Complete them at home
Take your time. The details you provide, from milestones to family history, directly shape how we care for your child. You can sign electronically by typing your name and date, or print and sign by hand if you prefer.

Send them back
Email your completed forms to appointments@texaschildneurology.com, or bring them with you to your appointment.

15 minutes early
Forms completed
Arrive 15 minutes before your appointment to finish the on-site portion of the new patient onboarding process.
30 minutes early
Forms not completed
No problem, life happens. Arrive 30 minutes before your appointment so you can complete the forms in our lobby without cutting into your child's visit.
What to bring to your first visit
- Your completed new patient forms, if you didn't email them ahead.
- Your physical insurance card. We scan it into your child's chart, and it carries information we need for claims, prescriptions, and any testing your doctor orders.
- A list of your child's current medications and dosages, plus any medication allergies.
- Dates and results of any prior testing, such as MRI, CT, EEG, or EMG, if your child has had them.
- Your pharmacy's name, address, and phone number.
