Location:
We are just north of the Medical Center of Plano, next to the
Chase Bank
building.
Office hours:
Our office hours are Monday -Thursday 8:00 - 5:00 PM,. Friday 8:00 - 3:00 PM
By appointment only. We close 11: 30- 1 :00 PM daily for lunch.
Phones are answered until 4:30 PM Monday - Thursday and until 3:00 PM on
Friday.
Cancellation:
We require 24 hours notice if you are unable to make your appointment. After
two failed appointments, the Dr. may release you from his care. If you are
over 15 minutes late, we may have to reschedule your appointment.
Prescriptions:
If you need a refill on a prescription, call your pharmacy and
request a refill for all non-controlled
drugs. All controlled drugs requests can be left on the pharmacy line or
emailed to
Pharmacy@texaschildneurology.com
There is a 10.00 charge for all controlled medications (triplicates).
The Pharmacy will contact
our office for approval. Refills are to be called in during office hours
only.
Financial policy:
We collect patient co-pays and/ or deductibles prior to your seeing the
physician. Please be prepared to pay. We accept cash, check, MasterCard and
Visa.
Medical insurance:
Your insurance may not cover the full cost of your charges, regardless of
insurance, payment remains your personal responsibility and is due before
you see the physician. We do not file
secondary insurance.
Forms:
All forms that need to be filled out will be done on Tuesday and Thursday
only, unless an urgent need exists.
Medical records:
We charge $25.00 for the first 20 pages and $0.15 per page thereafter.
The fee is collected prior to receipt of records and they are done on
Tuesday and Thursdays only. When requesting medical records, please contact
our office at least 5 days prior.
