Office hours:
Our office hours are Monday – Friday 8:00 – 5:00 PM.
By appointment only.
Phones are answered until 4:30 PM Monday – Friday.

We require 24 hours notice if you are unable to make your appointment. After two failed appointments, your physician may release you from his care. If you are over 15 minutes late, we may have to reschedule your appointment.

If you need a refill on a prescription, call your pharmacy and request a refill for all non-controlled drugs.  All controlled drug requests can be sent to your physicians Medical Assistant/Nurse.  (see Staff page for email addresses). There is a $10.00 charge for all controlled medications (triplicates). In addition, we need 48 hours to refill a controlled medication. The Pharmacy will contact our office for approval for non-controlled drugs. Refills are to be called in during office hours only.

Financial policy:
We collect patient co-pays and/or deductibles prior to your seeing the physician. Please be prepared to pay. We accept cash, check, MasterCard and Visa.

Medical insurance:
Your insurance may not cover the full cost of your charges, regardless of insurance, payment remains your personal responsibility and is due before you see the physician. We do not file secondary insurance.

Forms and Letters:
There is a $20.00 fee for forms and letters completed or certified by the physicians. The fee is collected prior to when the form or letter is released. Blank forms will not be accepted, parent’s portion must be filled out before being submitted to the physician. We require a three (3) business day turn around time for forms and a one (1) week turn around time for letters.

Medical records:
We charge $1.00 per page for the first 25 pages. And then $0.15 per page thereafter.
The fee is collected prior to receipt of records. When requesting medical records, please contact our office at least five (5) days prior. (see ‘Forms‘ for Medical Release.)
